Below are important forms that we typically need on file for each child. Some are for initial enrollment and others need to be updated annually or as needed. If you have any questions, please do not hesitate to speak with a staff member.

Complete form if your child has special medical/physical needs that should be addressed. A separate plan must be written for each condition.

Annual Maintenance Forms

Child Medical Statement

Each child must have an annual physical exam and show that their immunization are up to date. Complete one form for each child. This form should be completed annually for each student attending Hilltop.

Administration of Medication

Hilltop staff are not allowed to provide any medication to your child unless a form is on file with the child care center. This includes non-prescription medications like diaper rash creams. Please complete one form per child per medication.
